Healthy Savings Plus - Terms and Conditions

Healthy Savings Plus is NOT Insurance, nor an Insurance company; and does not pay any bills owed to Providers and/or Medical Facilities for the Healthy Savings Plus customer or customer’s dependents.

Healthy Savings Plus is a Healthcare Savings program that contracts with different networks in order to access savings on Prescriptions, Dental, Vision, Chiropractic and Medical Services. The Healthy Savings Plus customer will ONLY receive a savings on their healthcare needs when they utilize the correct network. If the correct network is not utilized, there will not be ANY savings.

Healthy Savings Plus does not guarantee any rates charged by a Provider, Medical Facility, Pharmacy or network.

Healthy Savings Plus customer or customer’s dependents are responsible for paying all Providers and/or Medical Facilities promptly.

Healthy Savings Plus reserves the right to cancel the membership of any customer who fails to pay medical bills from Providers or Medical Facilities promptly.

Healthy Savings Plus customer may cancel their purchased program at any time. If the program is cancelled within the first 30 days, the customer will receive full refund of charged fees minus any savings received from the utilization of the program. The initial $15 enrollment fee is non-refundable at any time.

The initial membership fee is due when the program is purchased. The customer’s bank or credit card draft date is 30 days from the purchase date. If the membership is purchased by an employer then the membership fees will be due on the 1st or 15th of the following month.

By signing the form the Healthy Savings Plus customer authorizes Healthy Savings
Plus to draft electronically from the customer’s bank account or credit card each month for the appropriate program fees.

If customer’s program is cancelled for non-payment, there will be no access to
savings with any network or Provider during the duration of cancellation. To reinstate any cancelled program the customer will need to contact the customer’s Sales Associate or the company directly.
